Before booking your appointment.


When you make an appointment booking, we strongly advise that you consider the following:
Appointments are 15 minutes long. If you require a Medical, Minor Surgery or double appointment (30 minutes) please phone 03 314 8504 to make your booking
Please use the optional facility of informing us for the reason for your visit - this helps us to ensure that you have booked the correct appointment.
If you are making a booking for a family member please ensure that they are registered with us.
Please be aware that children's immunisations, Travel immunisations, Cervical Smears are Nurse Clinic appointments and cannot be booked using the online service.
Book with your regular doctor - this is better for your continuity of care
If you do not require your booking please ensure that you cancel
at least 2 hours prior to the appointment.

How to Register

Come into the practice in person, and :

  • Bring Photo ID
  • You must also be enrolled at AMC
  • Have an email address (separate to any other registered patient), and
  • Be over 16 years old

Repeat Prescriptions and Test Results are available online. 

Bring your I.D. to the practice to register.

Access Online Services
  • Repeat Prescriptions

    • Enrolled patients can register to order repeat prescriptions online.
    • You must have prior approval from your GP before using this service. 
    • For your wellbeing, we do not provide repeat prescriptions unless you have been seen by a GP in the last six months.
    • Repeat prescriptions will take 24 hours to process.
    • There is a charge for this service - visit the fees page for current fees and charges.
  • Test Results

    • Enrolled patients can register for online services to access their test results on-line.
  • Who can use Online Services?

    • Enrolled patients
    • 16 years old or over
    • With a unique personal email address
    • Who have provided proof of identity
